JOB DESCRIPTION:
Position: Behavioral Health Provider Licensed/Associate Clinical Social Worker or Licensed/Associate Marriage and Family Therapist Department: Behavioral Health
Reports to: VSHWC Director of Behavioral Health
Pay: $32.
00 - $55.
00 Hourly, plus benefits
General Summary: By utilizing a collaborative, team-based model, provides assessment, intervention and treatment to individuals, families or groups for psychological, behavioral, cognitive or emotional challenges.
Provides services within the scope of practice of an LCSW/ACSW or LMFT/AMFT for the Valley Springs Health & Wellness Center.
Hours of Operation: Monday through Friday 0730-1730, 10-hour shifts available
Essential Job Functions:
Assess each patient's unique condition, combining information from medical diagnoses, clinical interviews and professional knowledge to establish an accurate picture of overall mental and emotional functioning
Consult and coordinate with the health care team members to assess, plan, implement and evaluate patient treatment plans
Develop comprehensive treatment plans, to include recommendations for auxiliary support groups, lifestyle changes and healthcare management
Provide individual, family and group therapy
Participate in a team-based, system of therapeutic care that provides support and strength to patients
Maintain high quality patient care by providing a safe environment through ethical and trauma-informed professional conduct and standards
Research local resources and eligibility requirements to determine goodness of fit for individual patients & assist clients with linkages to resources to help support their progress (transportation, housing, financial assistance, etc.
)
Collaborate with other service agencies and professionals in the mental health industry, such as psychiatrists, counselors, and school support staff to create the best treatment options for patients
Monitor symptoms and functioning, response to treatment plan, and need for revisions to treatment plan based on readiness to change, social and contextual factors, and level of engagement
Maintain clear, concise and accurate documentation within EMR/EHR
Comply with VSHWC privacy, security and confidentiality policies
Conduct peer review audits
Participate in community service and company-sponsored events
Attend all training sessions for the use of equipment, EMR/EHR, procedural functions, insurance training, staff meetings, OSHA, and any requested committee meeting
Comply with applicable code of ethics (by discipline - NASW, AAMFT, etc.
)
Other duties as assigned by the Director of Behavioral Health

Knowledge of relevant DSM-V and ICD-10 codes
Demonstrates competency in caring for patients of all age groups, including children, adolescents and elderly

Required:
Master's degree in Social Work, Psychology, Marriage and Family Therapy, or related field, received from an accredited institution
Licensed Clinical Social Worker, Licensed Marriage and Family Therapist, or eligible for licensure (registered AMFT/ACSW)
Personable, flexible, and compassionate personality
Successful interview with the Integrated Behavioral Health team
Ability to pass a criminal background check
